Partnership arrangements can be on a short, medium, or longer term basis and can include the provision of culturally or linguistic specific support, support coordination or case management, clinical or registered nursing services or assessments, or direct support services across funding streams that include but are not limited to:
- NDIS packages
- Home Care Packages (HCP)
- Commonwealth Home Support Program (CHSP)
- Transition Care Services (TCP)
- Short Term Restorative Care (STRC)
